ACL injuries like ruptures and non-contact tears of ligament are significantly higher in women than in men. Women participating in similar athletics as men are thrice more vulnerable to ACL injuries than men. Sports physicians and trainers often advise women athletes to adopt an ACL conditioning program. Recent research indicates gene variants in women could be responsible for higher incidence of ACL tears in women.

Various factors are responsible for this difference. Women have lesser flexibility of knee and hip. They also have a wider Q angle due to wider pelvis. Both together are responsible for easy susceptibility of women to ACL injuries. Further, women undergo numerous changes in muscle tension and hormone levels during menstrual cycles. Ligament strength is more in men than in women. Women are thereby more prone to ACL injuries and tears.

ACL injuries predominantly affect young athletes. Most ACL injuries occur due to athletes landing flat on their heels with knees set in a straight position. Force generated due to such landing travels directly to the knee through tibia, which is in a back slanting position. Forward slide of tibia is restricted leading to an injury in ACL.

Nonetheless, ACL tears are also common among people over the age of forty due to normal wear and tear of ligaments. A small fall or slip could lead to an ACL tear.
