Bank Loan Application Form &#8211; One Copy<\/p>\n<p>2. Last Three Months Salary Slips, Salary Credit Certificates, Income Tax Return, or Form 16 of the Assessment Year 2011-2012 \u2013 One Self-Attested Photocopy with the Original<\/p>\n<p>3. Last Six Months Statement of the Bank Account \u2013 Two Self-Attested Photocopies with the Original<\/p>\n<p>4. Residence Proof \u2013 Two Self-Attested Photocopies with the Original (any one of the following documents)<br \/>\n&#8211; Passport<br \/>\n&#8211; Latest Telephone Bill<br \/>\n&#8211; Latest Electricity Bill<br \/>\n&#8211; Latest Water Bill<br \/>\n&#8211; Income Tax or Wealth Tax Assessment Order<br \/>\n&#8211; Latest Insurance Premium Receipt<br \/>\n&#8211; Latest LIC Policy<br \/>\n&#8211; Address on the Letterhead of Employer (Only for the Government Employees)<\/p>\n<p>5.
